Season 16 - 8 Week Program

I, {name}, am committed to making positive changes in my health through my participation in my training with Kent. I understand that certain elements of this program can be physically and emotionally demanding, and that I may need to change various aspects of my lifestyle to achieve my goals. Big Kent's Promise instills that Kent is responsible for providing the coaching I am requesting and I realize his responsibility. I am responsible for my own participation in this program and for my own physical and emotional wellbeing. 

To conduct my business effectively and efficiently, the following terms and conditions apply:

Training sessions must be cancelled with a 24 hour notice, unless emergency or sudden illness occur.

• Please maintain communication with Kent. A lapse of more than 5 days will result in a lost time slot. However, client will still be responsible for paying remaining unpaid balance.
• Rescheduling a session time slot will be accommodated on a first come first serve basis. It is most important for a client to have a consistent schedule.
• If you need to reschedule occasionally that is okay, however, if it is on a consistent basis, it may result in the loss of your time slot.
• If you are more than 10 minutes late for a training session and have not notified Kent, you will be considered a NO SHOW, and your session will be cancelled for the day.
• Time slots cannot be guaranteed after lengthy vacations or time away without prior communication with Kent.
• It is likely that you will have a training partner. Kent can and will book other clients in your time slot if need be. We do conduct group sessions here at Big Kent's Strength and Fitness.
• If client decides to stop training, coming to sessions or makes the choice to quit, the said client is still responsible to PAY FULL BALANCE. There are NO REFUNDS here at Big Kent's Strength and Fitness Training FOR ANY REASON. Once contract is discussed and signed, you are fully responsible for the agreed upon payment.
• You are responsible for paying your monthly fees on time as scheduled herin this contract, however, rates are subject to change. You are responsible to PAY FULL BALANCE owed monthly. There are NO REFUNDS. If client chooses to discontinue training here at Big Kent's, said client MUST give a 7 business day notice in writing (email, text, letter) to Kent. If failed to do so, you will owe your payment on the scheduled payment date. 
• You are NOT to bring anyone that is not a part of Big Kent's Strenght & Fitness inside the facility without a health & safety waiver signed by said person. There will be a fee of $10 for any outside guest to enter the facility and train. If you are caught bringing a guest into the facility without a health & safety waiver signed, you will be subjected to a $200 fine and loss of your membership.
• All kids must be at least 5 years old to be in the facility. No child is allowed to enter into the facility without supervision.

As a condition of my enrollment, I accept full and complete responsibility for my own ability to healthfully participate in the program. I understand that participation and use of instruction, programs, activities, services, facilities, and equipment provided by Big Kent's Strength and Fitness is potentially hazardous.

I hereby release Big Kent's Strength and Fitness, it's owner, trainers, and all others from any responsibility or liability from any injury, damage, or any loss whatsoever, including those created by their own negligence. 

I have read and understand the above contract and that is a total release and waiver of liability.
